1. Print this registration form.
2. Enclose the following document(s): a copy of this
registration form, a copy of the sales receipt, and the
original UPC code from the product packaging taped
to the rebate form.
3. The UPC symbol MUST be taped to the AFFIX UPC
CODE HERE box to ensure timely processing of your
rebate. Do not staple the UPC code to the registration
form or leave the UPC code loose in the envelope.
4. Send all of these items to the address at the bottom of
this registration form.

Offer Terms: This mail-in rebate card offer is valid only with purchases between July 14th, 2016 and July 20th, 2016 by an end user
customer on Newegg DIYPC D780-BK Black Computer Case.
Rebates will be issued in the form of a prepaid card. To qualify for this rebate card, only the actual purchaser of the qualifying product may
participate. End user must keep copies of all materials sent. Materials received become the property of Newegg and will not be returned.
Please allow 6-8 weeks for receipt of rebate card. Newegg is not responsible for lost, late, incomplete, damaged or misdirected mail. Use of
